One-time Initial Enrollment fee per family unit: $150
There is a 5% discount for members paying one year in advance and a 3% discount for paying 6 months in advance.
Other Fine Print:
There are no long-term contracts or commitments.
To cancel a membership, please request in writing 30 days prior to the date you wish to stop. Membership rates are not pro-rated in the final month of membership.
Charges are billed on the day of enrollment, and on the same day each month prior to services being rendered.
To prevent frequent or repeat membership cancellations, a $450 re-enrollment fee will be charged.
We reserve the right to charge $50 fee for each no-notice, missed appointment; that appointment time could have been given to another person, even with same day cancellation notice.
PLEASE NOTE: HMO insurance plans, TRICARE PRIME and Medicare Advantage plans require patients to have a primary provider in their network in order for them to pay for ancillary health services like x-rays, medications and specialty visits. These plans do not pair well with a Direct Primary Care membership practice.
